THE NEED FOR A COMMON LANGUAGE FOR COMMUNICATING ABOUT MEDICATIONS WITH POLYPHARMACY PATIENTS
Patients typically don’t know the names of their medication, and often generic drug with changing brands are used when prescriptions are renewed. There is no commonly available and reliable solution for providing information about the physical characteristics of the medications to the HCPs.
Patients with chronic heart conditions often have comorbidities, and are using more than 10 prescribed medications every day (excluding OTC), multiple times a day, and often in different periodicity, and sometimes with further instructions (and after a meal). Regular telephone consultations with patients is an effective approach to improve patient outcomes, and provides opportunities to adjust their medication without hospital visits. This makes it difficult to talk about the medications with the patient.
